Choreography Team Info Submission

To help us prepare for your upcoming choreography sessions, we ask that gym owners complete one form per team. Please ensure all athletes, including single athletes, are listed in their respective team form.

If your program has cross-over athletes, please complete the cross-over form separately. Cross-over athletes receive 50% off their second routine, with the discount applied to the least expensive package where applicable.

Please note that dates discussed are not locked in until a 50% deposit has been received by the due date provided by Passion Performance. Any arrangements or changes to team numbers can be amended prior to final payment. Final payment for choreography must be made 24 hours before the final choreography session.
